The fair creature had the look of an adult woman, only she was the size of a child, dressed in the finest dress. She had the transparent wings of an insect, and antennas to match. She was very beautiful, in her hand she carried a small wand and she had a faint glow around her, as a star on the night sky.

A fairy queen is the matriarch of a fairy tribe, acting as the leader and protector of her clan and almost as a mother to the smaller fairies. While playful and carefree most of the time, they have a more grown-up personality then the normal whimsical fairies and have a great patience towards the childish behavior of their sisters. Fairy queens are well-respected by all creatures living around them, and anyone who proves themselves to a fairy queen will not only win the respect and friendship of the fairies, but also of all the fae creatures in the woods.
It is not uncommon for fairy queens to have a friendly relationship with mortals outside their fey tribe, usually these mortals are druids or other friends of nature, but sometimes they take it upon themselves to watch over and protect a child as its fairy godmother. Fairy queens are known to aid good adventurers who respect nature and the creatures in it, and they are said to even grant wishes to people who prove they are worthy and have a good heart. While they live as sole rulers of a fairy clan, whole groups of fairy queens sometimes gather to discuss problems brewing near their homes. They also make lengthy friendships with larger fey, like nymphs or dryads, and other beings of the forest like treants and unicorns.
A fairy queen stands roughly the same size as a gnome at 4 feet tall and about 35 pounds heavy. They speak Sylvan, Common, Elven, Draconic, Gnome, Goblin, Auran, Celestial and Druidic, and they may know more languages as well.

Combat

A fairy queen always try to neutralize her opponents with her spells or spell-like abilities in combat, preferring misdirection and deception over direct confrontation, and leave the target sleeping or disoriented while she and her clan hide somewhere safe. But if her fairy sisters are threatened, a fairy queen will attack with a surprising rage, shooting eldritch fire and magic missiles at the target. They make good use of their ability to become invisible to phase in and out of sight to get the element of surprise.

Greater Invisibility: A fairy queen can use Greater Invisibility on herself at will, and she can suppress or resume it as a free action.

Spell-like Abilities: Caster level 12th, DC's saves are Charisma-based.

Spells: A fairy queen casts divine spells just like a 10th level druid, and are able to cast spells from both the druid and cleric spell-lists and the domains Animal, Good, Fey, Luck, Magic and Plant.

Typical spells prepared;
Lvl 0: Cure Minor Wounds (×2), Detect Poison, Guidance, Resistance, Virtue
Lvl 1: Bless, Doom, Obscuring Mist, Remove Fear, Sanctuary
Lvl 2: Aid, Barkskin, Heat Metal, Lesser Restoration, Summon Swarm
Lvl 3: Cure Serious Wounds, Invisibility Purge, Protection from Energy, Quench
Lvl 4: Flame Strike, Holy Smite, Iron Butterflies
Lvl 5: Call Lightning Storm, Dispel Evil

Summon Fey: Four times per day, a fairy queen can summon other kinds of small fey. She may choose to call 4 grigs, 4 fairies or 3 pixies (that does not know Otto's Irresistible Dance).

Wish: Once per week, a fairy queen can choose to grant a wish to a good mortal. This duplicates a Wish spell cast by a 20th-level sorcerer.

Skills: A fairy queen has a +2 racial bonus to Search, Spot and Listen checks. She also has a +4 racial bonus to Craft (sewing), Knowledge (arcana & nature) and Spellcaft, and a +8 racial bonus to any Perform check.
Fairy queens have been in a lot of poems and writings, like Shakespeare, but not so much in actual folklore. Still, I thought of the idea as pretty cool so I made stats for them.

Naturally those that can use magic that can dispel buffs like dispel magic and maybe antimagic they are a fairy queens worst fear I mean having your cast success ratio sent down to 20%?! Now that would force a fairy queen to attack physically
